Local chapter corporate sponsor membership is a support function open to individuals and organizations that provide products and/or services to PAHCOM members. PAHCOM membership consists of health care workers in all specialties and allied health science professions throughout the United States. Local chapter corporate sponsor membership provides an opportunity to promote outstanding products, supplies and/or services available to benefit, enhance and support physician office practices. I agree to promote the professionalism of The Greater Chicagoland Chapter of PAHCOM and to further support the association by offering our products, services and/or supplies to Greater Chicagoland Chapter members and their practices.

Sponsorship fees are paid in advance yearly. Fees are due by Janurary 1st of each year. All Sponsors, we request that you provide the meal for one of our chapter meetings. If you have a conflict since signing up for a particular month, please let me know so that we can make the change. First come basis. For all other sponsors, we ask that you donate $100.00 toward our Holiday Party in December if you do not provide a dinner during one of our monthly meetings. All new sponsors email a one page ad in word or pdf and a logo in .jpg or .gif format to website@gcpahcom.com after filling out this form.
